Eligibility:  Medicaid insured children and adults; uninsured adult residents of Hampton Roads cities with household incomes below 200% federal poverty. Proof of income & residency are required along with a current federal tax return, picture ID, and social security card.

Fees: $20 annual fee and $30 per visit.

Eligibility: Residents of South Hampton Roads. Children enrolled in the school lunch program are automatically eligible for services. Proof of income & residency required.

Fees: Most insurance plans, including Medicaid, Medicaid HMO's and FAMIS are accepted. Uninsured individuals pay sliding scale fees according to income level.
