What is a Free or Charitable Clinic? Clinics are safety-net health care organizations that utilize a volunteer/staff model to provide a range of medical, dental, pharmacy, vision and/or behavioral health services to economically disadvantaged individuals. Such clinics are 501(c)(3) tax-exempt organizations, or operate as a program component or affiliate of a 501(c)(3) organization.

Entities that otherwise meet the above definition, but charge a nominal/sliding fee to patients, may still be considered free or charitable clinics provided essential services are delivered regardless of the patient's ability to pay. Free or charitable clinics restrict eligibility for their services to individuals who are uninsured, underinsured and/or have limited or no access to primary, specialty or prescription health care. Eligibility: Uninsured, at or below 200% FPG, not eligible for Medicaid or Medicare and must be a resident of the cities of Suffolk or Franklin, or the countries of Isle of Wight or Southampton.  Financial screenigns are done every 12 months.
Proof of Income Requirements: Picture ID, Proof of Social Security Number, IRS 1040, W-2 and 4 weeks of recent consecutive paystubs (if employed), SNAP/food stamp letter, disability letter, proof of alimony or child support, letter from homeless shelter, letter of support from whomever provides you with food and shelter for those with no income (these requirements are only if applicable) Proof of residency such as current mail or a bill or a bank statement (Dated within last 60 days)
