Proof of Income Requirements: Picture ID, Proof of Social Security Number, IRS 1040, W-2 and 4 weeks of recent consecutive paystubs (if employed), SNAP/food stamp letter, disability letter, proof of alimony or child support, letter from homeless shelter, letter of support from whomever provides you with food and shelter for those with no income (these requirements are only if applicable) Proof of residency such as current mail or a bill or a bank statement (Dated within last 60 days)

Services: Primary Care, Women's Health (excluding birth control), Mental Health, Ophthalmology, Endocrine, Pharmacy, Pulmonology, ENT, Rheumatology, Nephrology, Hematology and Dental.  Referrals to VCU/MCV for other specialty care.  Transportation not provided. 
Fees: Medical $5 donation per visit; Pharmacy $4 donation for each supply of medication; Dental Mandatory $10 administrative fee must be paid prior to making dental appointment which is non-refundable if patient does not show up for their appointment without cancelling or rescheduling at least 24 hours in advance.  This fee must be paid prior to making any other appointments, utlizing ANY clinic services or picking up medications.
