Proof of Income Requirements: Picture ID, Proof of Social Security Number, IRS 1040, W-2 and 4 weeks of recent consecutive paystubs (if employed), SNAP/food stamp letter, disability letter, proof of alimony or child support, letter from homeless shelter, letter of support from whomever provides you with food and shelter for those with no income (these requirements are only if applicable) Proof of residency such as current mail or a bill or a bank statement (Dated within last 60 days)

This fee must be paid prior to making any other appointments, utlizing ANY clinic services or picking up medications.
